Life Insurance

Amphi provides all benefits eligible employees, under the age of 70, with $25,000 in term life insurance and accidental death and dismemberment (AD&D) benefits at no charge.  Insurance is reduced for employees over age 70.  

It is important to keep your beneficiary up to date.  Please provide the name, relationship, full address, date of birth and SSN (if known).  In the event of your death this information is extremely important is trying to locate your beneficary and pay the proceeds of your life insurance.  Beneficiary imformation is in the Benefits portal.

  • Insurance ends at termination or if you are no longer benefits eligible.
  • Our policies are portable - you may contact The Standard, within 31 days of your insurance ending, to obtain rates and policy information.
  • If you are totally disabled and under age 60, you may request a waiver of premium. Please see the Life Insurance Plan Summary (above) for additional information 

Optional Additional Term Life and AD&D Insurance

  • Provided through The Standard
  • Additional insurance is available for amounts of $25,000, $50,000, $100,000 and $200,000
  • Evidence of insurability required for any additional insurance
  • An additional $25,000 in coverage is a flat rate of $6.95 per month
  • Amounts other than $25,000 have rates that are age based and will increase as you age

Optional Permament Life Insurance

  • Provided through Texas Life
  • Rates are fixed at time of purchase
  • Portable - you are able to keep the insurance, at the same rate, if you leave Amphi or are no longer benefits eligible

Travel Assistance

The Standard also provides employees with free Travel Assistance. This program provides assistance, for trips over 100 miles, for emergencies such as transportation, legal, medical, and pre-trip assistance. The service assists the employee in locating the necessary assistance.  Click here to find out more information


Disability Insurance

  • If you are ill or injured and will be out for over 45 days, apply for short term disability as soon as you know you will exceed 45 days of absence due to disability
  • If you have been out for several months and anticipate being out longer, apply for long term disability (all benefits eligible employees are provided long term disability insurance through the ASRS)

Short-term Disability

Provided through Hartford.  Short-term disability insurance is provided, free of charge, to all benefits eligible employees. Benefits include coverage at 60% of pay, for a period of up to 20 weeks, if qualified and approved. There is a 45 calendar day waiting period from date of disability (you may use sick time, personal time or vacation time during the 45 days). The maximum weekly benefit is $1,500, regardless of salary. Premiums are included in your taxable wages so that any benefits paid do not have taxes withheld.  (Academic year employees who are approved for short-term disability, will only receive the benefit during the time they would normally work.)

Short Term Disability Summary Plan Description



Long-Term Disability

Long-term disability insurance is provided for all benefits eligible employees. This insurance is part of the benefits through Arizona State Retirement System (ASRS) and contributions are mandatory. Both the employee and employer contribute.  The employee contributes .25% of their afater-tax pay for this benefit.  Benefits provide income at 66 2/3% of your pay, after 6 months of approved disability, until disability ends, retirement, or death. If you are approved for Long Term Disability, you continue to accrue credited service towards your retirement. You are also eligible, based on years of service, for a subsidy (from ASRS) to help defray the costs of medical insurance if provided through the District, District COBRA, or ASRS.
