Transcript Request
If you are a current student and need to send a copy of a transcript to a college/university or scholarship committee, please see your counselor.
If you have attended or graduated from Amphitheater High School and would like a copy of your transcript sent out, here are the procedures:
1. We need a written request with your name during enrollment; last year attended and date of birth.
2. There is a service fee of $2.00 per transcripts (payable in money order or cash only)
3. Please include the name and address of where the transcripts need to be sent.
4. Request's are processed in two working days
