Registration for 2012 - 2013  and 2013 - 2014

Dear Parents/Guardians;

Welcome to AMS. Pre-registration packets for current 6th and 7th graders have been sent home with your child. Please return by May 3rd; the class that returns ALL their forms first will win a pizza party. Copies of the forms are available online:

Replacement forms are also available in our temporary office at Prince Elementary, 125 East Prince Road.

Only those students who return their packet will be placed on the Fall 2013, class rosters.

Students are not considered "REGISTERED" until they come in during assigned days in the summer to pick up their schedule and have their I.D. picture taken.

Registration Hours: 8:00 am - 3:00 pm in the Prince Mustang Room (We are tentatively scheduled to be closed during lunch from 12:00 - 1:00 pm.)

  • 6th grade registration: Monday, 7/22/2013
  • 7th grade registration: Tuesday, 7/23/2013
  • 8th grade registration: Wednesday, 7/24/2013
  • All grade registration: Thursday, 7/25/2013

If the child is a returning Amphi Middle School (AMS) student, please bring:

1) Proof of Residency: Arizona State Law requires that parents/guardians provide proof of residency EACH YEAR when registering their student. We can accept a copy of a current month’s lease, mortgage, tax bill, gas, electric, water, telephone, or cable bill with a parent name and address on it.

2) Immunization Record –

  • 6th grade Tdap (Tetanus, Diphtheria & Pertussis)
  • 7th grade Proof of Varicella Vaccine is required if there is no history of Chicken Pox.

If the child is new to Amphi Middle School (AMS) you will need, in addition to the above:

3) Birth Certificate

4) Name and address of previous school.

Students need a physical every year, for participation in after school sports, information is available in your registration packet or you may stop by the AMS Office.

Our attendance boundaries are:

  • South Orange Grove
  • West of Campbell Avenue (odd numbers)
  • North of Grant Road
  • East of Oracle from Orange Grove to Wetmore and East of Stone from Wetmore to Grant Road (even numbers)

 

Our “feeder” elementary schools are F. O. Holaway Elementary, Helen Keeling Elementary, Prince Elementary (part), Rio Vista Elementary (part) – (part = according to address or boundaries.)

Our students, upon promotion, attend Amphitheater High School at 125 West Yavapai Road 85705, (520) 696-5340.
