1. Northwest Medical Center Urgent Care; 2945 W. Ina Road., 219-6616, or 13101 N. Oracle Rd., 818-2000. Fee $35.00 9:00 am – 9:00 pm at both locations. Physicals on walk-in basis.

2. Keeling Health Clinic; 435 E. Glenn, 696-6969. Fee $25.00. Monday thru Friday, 8:00 am – 5:00 pm. Physicals by appointment only.
3. Pusch Ridg Family Medicine; Kimy Charani, D.O., Ted Crawford, D.O.; 7390 N. Cholla Blvd. 825-3547. Fee $35.00. Physicals by appointment only.
4. Concentra Medial Centers: 2005 W. Ruthrauff Rd., Suite #111, 293-7250; 3402 E. Broadway Blvd;881-0050; or 4600 S. Park Ave., 889-9574. Fee $30.00 Physicals for students age 14 and over only. Physicals by appointment only.
5. Family Practice: Steven B. Wallach, D.O.; 3005 W. Ina Road; Suite#123, 575-9110. Fee $35.00. Physcials by appointment only. Same day appointments available.
6. NextCare; 6238 E. Pima, 290-0022. Fee $45.00 – Cash, credit, debit. Monday - Friday, 8:00 am – 8:00 pm, Saturday and Sunday 8:00 am – 4:00 pm Physicals on a walk-in basis. Must arrive one hour before closing.
7. El Rio Santa Cruz Health Center; 839 W. Congress St., 670-3909. Must be a member before making appointment for physical. Sliding fee scale based on family income. Proof of residency, 2 months income verification, birth certificate, and social security card are required.
8. St. Elizabeth of Hungary Clinic; 140 W. Speedway Blvd., Suite #100, 628-7871 (ask for registration). Fee based on sliding scale. Must be a patient before physical appointment.
9. Sunnyside Medical Center; 3681 S. Palo Verde Road, 750-8855. Fee $40.00 Monday – Friday, 7:30 am – 11: 00 am and 1:00 pm – 4:00 pm. Physicals on a walk-in basis.
10. A New Creation Women’s Clinic; 5601 N. Oracle Rd., #101, 293-1117. Physicals for girls only. Will accept co-pay with insurance or $35.00 without insurance. Appointments required.
11. Kino Teen Center; 2801 E. Ajo Way, 740-4600. Ages 12-21. Accepts many insurance plans including AHCCCS. For uninsured, physicals are $20.00 - $40.00. Wednesday and Friday only, 2:00pm - 5:00 pm |
