Welcome to La Cima Cardinal Athletics. The vision of the La Cima athletic department is to provide opportunities to learn skills, practice sportsmanship and compete in a controlled environment against a variety of skilled athletes.

Important "Physical" Information
Just a reminder, all La Cima athletes are required to turn in a Physical form, completed by a doctor. Remember, no athlete will be allowed to practice or play until all paperwork has been filled out and a physical is on file in the office. Thanks for your attention to this issue. Let's have a great season!
For returning athletes: All physicals dated after March 1st of 2007 are good for all of this school year. ___________________________________________________________________________

Before an athlete may play or even practice several forms must be filled out. Following is a list of what is needed:
1. A current physical on file in the office. This physical is good for one year and must be completed by your doctor on an Amphi physical form. Any physicals completed on or after March 1st, 2004 are good for the balance of this school year.
2. A signed "Consent for Emergency" form. This states that the student has your permission to play sports at La Cima. It is good for one year.
3. A signed "Acknowledgement of Rules and Terms for Interscholastic Participation" form. This describes not only the rules for the student-athlete but also for all parents and spectators. This is good for one year.
4. A signed "Emergency Card" that will be given to the coach in case of an emergency at practice or at a game. This is good for one season.
5. Pay a $15 fee (per sport). All students on the "free and reduced" lunch program, qualify for a fee waiver and need not pay the $15.
