Immunization Requirements for Admission:
DTP, Polio, MMR, Hib, Hepatitus B, Varicella (Chicken Pox)

Starting this school year, August 2008, all students who are 11 years of age and entering into 6th grade are required to have two additional immunizations for school attendance.

1. Tetanus Diphtheria Acellular Pertussis (Tdap) Booster
2. Meningococcal (Menactra) Vaccine

Please take your child to his/her physician or call the Pima County Health Department at 243-2850 for further information about times of the next available vaccination clinic.

Medicine

Medicine must be prescribed by the child’s doctor.

Prescription drugs must be in the original pharmacy container, labeled with name, date, medication, and dosage.

Nonprescription medication such as cough or throat lozenges, Tylenol, aspirin, ibuprofen, topical creams, and sunscreen may not be given without written authorization from the doctor.

Authorization must be on file in the health office.

It is against District policy for children to carry any medication on them. Asthma inhalers can only be carried by students if indicated by their doctor for school use.

All medication must be checked in at the Health Office.

Emergency name and phone numbers are kept in the Health Office. Please keep these updated.

Students will only be released to individuals on the Health/Emergency card.
