Heading: All assignments and journal entries are to carry a heading on the upper right hand corner, above the top line of that entry. Journal entries must have the date and assignment name.

Paper assignments must have a formal MLA format heading for title pages and page headings. The heading should be at the top of the first page.

The heading is to include:

Student name******Jennifer Barnes
Grade level/Subject*******7th Grade Language Arts
Class Period******Sixth Period
Teacher's name******Mrs. Wyckoff
Arizona Standard code******E-R5
The date the work is handed in******Date

The Title

The student's last name and the page number should appear on the top right corner of each subsequent page. Example: Barnes 2

* Credit will not be given for unidentified work. Work handed in on the due date with no name will be counted as late, and five workhabits points will be deducted from the student's possible 100 workhabits points for the quarter.

* Grade Print-outs are sent home to be signed every two weeks, usually on the first day of the week.

* All students have the option of having their grades posted on this secured website, so parents or students can check the student's grades at their convenience. Students with permission forms turned in and with passwords may also check their grades on available computers at school. These opportunities to monitor grades are the student and family responsibility. As soon as the signed permission form for posting grades is returned to me, I will enter the password and post the grades for that student.

* Students who opt not to have their grades posted online will be able to get additional updates on their grades only if they carry and get signed daily gold sheets and if they arrange to meet with me after school, at 4:00,during ACHIEVE. I will not be available to look up and give students current grades during class or during the school day.

* Assignments are to be written in the student's own words, in complete sentences. Students must be prepared to justify or explain their definitions and statements.

* When students are absent, they are to keep their assignments current by getting notes from a friend. Each student is to refer to the large classroom calendar for due dates.

* All work is to be completed in pencil or erasable pen. Scribbles, whiteout, striking through words will not be accepted. In order to receive full academic and workhabits credit, student must rewrite and resubmit incomplete or unacceptable work (complete, correct, and comprehensive) before the end of the day. Work may be handed in after ACHIEVE to receive same day credit.

* Cornell journal notes will be graded weekly, and notes will be used as a basis of study for all exams.

* Students are responsible for filing final, graded samples in their three-ring portfolio binder and placing the binder back in the gray cabinet for safe keeping. Lost or misplaced items are to be rewritten. Students are therefore encouraged to save final drafts to a disc and in a file on their computer at home.
