A medication permit must be signed by the parent/guardian for the medication to be taken at school.
Prescribed medication must be in the original container with dispensing information and kept in the Health Office locked cabinet, unless a written note from the doctor requests otherwise.
All over-the-counter medication must also have a note written by the doctor specifying what the medication is for, how much is to be given and at what time.
This is for the safety of your child and the other students. |
