We have two registered nurses on staff at Cross Middle School (CMS). Loy Mentzer works Monday & Tuesday and Roni Dvoskin works Wednesday thru Friday. We serve as the health professionals for the school and provide the following services:
Illness,
injury assessments and interventions
Health
assessments/participation in Individualized Education Plan Development
Identification,
assessment, planning, intervention, & evaluation of students health concerns
Screenings
for health factors impacting student education such as vision& hearing
Activities
& education to promote healthy lifestyles
Chronic
disease management & education
Medication
administration & monitoring
Assessment
& referral for students with mental health needs
School/community/health
care provider liaison
Occasionally the nurse’s duties may require being off campus. If an emergency arises with a student during such a time, the office staff or administration will handle the situation, call paramedics if necessary, and notify the parent/guardian.
Important
Immunization InformationAccording
to Arizona State Law, all students must be in compliance with current state
regulations concerning immunizations for preventable diseases unless the
parent/guardian has signed a valid exemption form which will then be kept on
file in the health office. A new
immunization requirement for incoming 7th & 8th grade
students is the chicken pox (varicella) immunization. If a student has had the
chicken pox disease then they do not require the varicella immunization,
however, parents are asked to fill out a survey form documenting when their
child had the chickenpox disease. If

Students will not be enrolled until documentation (i.e., school record,
medical records, international travel immunization record, etc.) is provided.
Parent’s recall is not acceptable.
Pima County Health Department offers low cost immunizations for children.

ALL medications brought onto campus, including over the counter drugs such as Tylenol, Advil, Midol, etc. must be prescribed by the student’s physician and stored in the CMS Health Office locked cabinet. Students are not allowed to have in their possession any medications with the exception of asthma inhalers & epi-pens. These can be carried by the student only if the district’s Permit Form , signed by the parent & verification of the prescription, is on file in the school health office every school year.
Prescription
Medications must be:
Brought
in by the parent/guardian
In
the original pharmacy container with proper labeling
Accompanied
by the district’s Medication Permit Form signed by the parent/guardian stored
in the CMS Health Office locked cabinet.
Non-Prescription
Medication must be:
Brought
in by the parent/guardian in the original, unopened container accompanied by the
district’s Medication Permit form signed by the parent/guardian and physician
and will be stored in the CMS Health office cabinet.
