Request Procedures

Return the form below by Email, mail or fax ~ All requests must include a legible photocopy of I.D. or Notarized Form.

Record requests are typically completed within 2 working days. Please note that during high volume periods such as yearly registration that requests may be delayed. Registration is typically the first couple weeks before and after the start of school. During this time we will attempt to complete your request as soon as possible, up to a maximum of 10 days.
