Registration Process

If you live in the Mesa Verde attendance area, we ask that you fill out a Kindergarten Registration Packet.

Be sure to download the file to your computer first, fill out the registration and then save. Please email your registration form to mesaverde@amphi.com. Give us a call if you are unable to fill the form out via computer and need a paper copy! 

When we have received your registration by email, we will call you to set up an appointment.  This appointment will be in person at the Mesa Verde Office (1661 W Sage) and will finalize the registration process.  At this appointment you will be asked to bring your child’s: 

• Original Birth certificate 
• Shot/immunization records 
• Proof of residency (utility bill, lease agreement, driver license, etc.)

Do you live outside the Mesa Verde boundaries?

If you do not live in the Mesa Verde Attendance Area, please fill out the Open Enrollment Form and make Mesa Verde your first choice.  You are not required to make a second and third school choice, but please select all the schools you are applying for in your preference order.  

Tell your friends and family!

If you know of any other families in the area with a child who will be starting kindergarten, or who attended kindergarten at a private school and need to register for 1st grade, please pass this information on to them.
