• Employee supplemental life maximum of $750,000 and guaranteed issue limit of $250,000*.
• Spouse supplemental life with a maximum of $250,000 and guaranteed issue of $50,000*.
• Child supplemental life with guaranteed issue in $10,000, $15,000, or $20,000 amounts^

*Guarantee issue amounts are only applicable to newly benefits eligible employees. Employees with existing benefits eligibility must complete the Evidence of Insurability process in order to be approved for increases to coverage amounts. Newly benefits eligible employees must complete the Evidence of Insurability process in order to qualify for amounts above the guaranteed issue amounts.
