      Thank you for choosing Copper Creek Elementary School!  You are going to love it here.  Copper Creek is an "A" rated school by the Arizona Department of Education, and we are focusing on personalized learning to see the growth that every child can make each year.  Parents are a part of this journey!  We meet with parents each semester to give updated information on your child's strengths and growth areas. 

      On this page, you will find information about registering/enrolling your children for grades K-5. If you would like to register for preschool, please go to www.amphi.com/preschools.

     
     

    We are looking forward to a wonderful 2024-2025 school year.  Bethani Gregg, from the school office, will be happy to answer any questions you may have.  Our Health Assistant will assist you with questions or concerns you may have about your child’s immunization records.

    The following are required in order to register a student:

    • Birth Certificate - original or certified copy
    • Immunization History - complete immunization record
    • Residency Documentation Form and Proof of Residency - Please see attached list of approved documents

    The following forms and items must be completed on-line as a part of the registration process:

    • STUDENT REGISTRATION FORM:  Complete the entire form, including your signature, and be sure to list your child’s legal name.  The first contact will be the primary and first emergency contact.  Please list all other contacts in the order you would like them to be notified.
    • SCHOOL HEALTH AND EMERGENCY CARD:  (Complete both sides of this card.)  Emergency contact information is vital when an emergency arises.  Please list someone other than yourself we can contact in an emergency; we will always try to contact you first.  (If for some reason the information changes during the school year, call the office and we will gladly update your child’s card.)
    • PARENT HOME LANGUAGE FORM:  This school district form is required to be in every student’s folder.  Please sign and answer all questions on it.
    • MCKINNEY VENTO FORM:  Please return this form by answering the two questions and then following instructions.
    • REQUEST FOR STUDENT RECORDS:  Complete this postcard, if student has had any prior schooling.
    •        CUSTODY ISSUES: If there are any issues (custody/restraining orders) we need to be aware of, please include this with your registration. Without the documents on file at the school, we will be unable to enforce any custody issues.
