For families with existing students:
If you are adding a new student (and have existing students in an Amphi school):
- Birth Certificate
- Immunization records (most recent)
If the student you are adding is in High School and you are withdrawing from a non-district school, please provide:
- Current transcript
- Withdrawal form
For families new to the District (with no students currently enrolled in an Amphi Schools):
- Birth certificate
