           To simplify the registration process, we have available to parents a fillable online PDF version of our registration packets for your convenience. Please review the Registration Checklist included in the packet for important information on documents and forms required for submission.

          Listed below are our packets for NEW and RETURNING students. Please complete the appropriate packet, print, sign where required, and return to the school office. Thank you!

    Returning Student Registration Packet

    Documentation Required:

    • *Residency Form
    • *Proof of Residency – Mandatory - (i.e. utility bill, cable or phone bill, lease or rental agreement, mortgage papers)

                         *Required ONLY if address has changed.

    New Student Registration Packet

      

    Documentation Required for New Student:

    • Original Birth Certificate
    • Current Immunization Record – Required to start school
    • Proof of Residency – Mandatory - (i.e. utility bill, cable or phone bill, lease or rental agreement, mortgage papers)
    • Guardianship papers, if student is living with someone other than parent listed on the birth certificate.
    • Individual Education Plan (IEP) if applicable
    • Withdrawal Form from prior school
    • Request for Records Postcard (you will complete this when you return packet to the school.)
